Video Summary
The webinar examined the 2018 Florida International University pedestrian bridge collapse, which killed six people and injured several others. The unusual, non-redundant concrete truss was moved into place before completion. After installation, severe cracking developed near the north end, especially around nodes 11–12. Despite repeated warnings and cracks large enough to expose significant distress, the engineer of record deemed the structure safe. Re-tensioning diagonal post-tensioning bars shortly before the collapse increased horizontal forces and contributed to the failure.

The NTSB identified several contributing causes: major design errors and underestimated shear demands, an inadequate and non-independent peer review, failure to recognize the seriousness of the cracking, and failure to close the road beneath the bridge. Investigators found that the materials and installation generally met specifications.

Key lessons include conducting thorough independent peer reviews, analyzing every construction stage, treating significant structural cracking as an urgent safety issue, protecting the public, and questioning assumptions—even when they come from experienced engineers. The speaker emphasized humility, decisive action, and prioritizing public safety over schedule, cost, or professional deference.
